Mastoplasty: types, interventions and objectives of breast plastic surgery

Breast augmentation

It is the most widespread, particularly in the cosmetic surgery sector and consists in the insertion of soft breast prostheses made of a silicone shell and filled with silicone gel to enlarge or reconstruct the breast. In some cases it is possible to use prostheses filled with hydrogel , saline solution (salt water) or other materials In addition to the hemispherical prostheses, used in additive procedures, there are new “anatomical” prostheses in the shape of a third of an egg.

The surgical act begins with the incisions, necessary to reach the area that will become the seat of the prosthesis or on the inframammary sulcus, or in the armpit or along the perimeter of the areola. After a thorough examination, the surgeon will identify the technique that best suits the needs of each patient.

After the incision, the surgeon creates the pocket where the prostheses will be placed and cauterizes all the small blood vessels cut in these phases. The prosthesis is slid into the surgical pocket and the drains are positioned. In all these phases, great attention is paid to the integrity of the mammary gland in order to maintain its physiological function and allow the woman to breastfeed. The tissues are then sutured first in depth and progressively along the more superficial planes, in order to distribute the forces evenly. The operation ends with the dressing and bandaging of the breast with compressive elastic bandages. The compression contains any edema that normally forms following the surgical act and promotes the correct settlement of the breast tissues. The bandage is usually removed after about a week.

Breast reduction

It is a cosmetic surgery to reduce the volume of the breasts due to excessive adipose or glandular tissue. In addition to the aesthetic purpose, breast reduction often has a functional purpose, because it solves the curvature of the spine, the grooves of the shoulders, the maceration of the skin, all problems caused by the excessive weight of the breasts.
The operation is performed under total anesthesia: the surgeon cuts along the prepared drawings, detaches the gland from the muscular plane and then removes the foreseen glandular / cutaneous excess. What remains of the mammary gland is remodeled and the areola and nipple are transferred in central position. Two drains and a skin suture complete the surgery. If used, the drains are removed after 24 hours and the patient can be discharged and antibiotic therapy continued. The sutures are removed after 7-10 days.

Mastoplessia

By mastoplessia we mean a surgical procedure of reconstructive training, used to lift the breasts when there is a loss of tone.
Loss of firmness usually occurs with advanced age, due to atrophy, mammary gland hypertrophy, breastfeeding and pregnancy. The intervention is also chosen in case of significant weight loss.
After an anesthesia, which can be local or total, the surgery takes place through initial incisions and with subsequent removal of the excess skin, providing a lifting effect to the breasts.
